Jones Chiropractic Clinic offers our patient forms online so they can be completed in the convenience of your own home or office, before you even arrive to our chiropractic office.
Download the necessary form(s), print it out and fill in the required information.
Fax us your printed and completed form(s) or bring it with you to your appointment.
After you've completed your forms, Dr. Jones will take time to listen to your concerns. He will take x-rays of your chief complaint area. Dr. Jones will then review your x-rays and discuss with you his findings. You will then be placed on our motorized intersegmental traction table to aid in your flexibility and make adjustments easier for Dr. Jones. After your relaxing time on the table, Dr. Jones will make the necessary adjustments and apply ultrasound to the area, if necessary. At this point, if you need to come back for further treatment, we will be available should you wish to discuss payment options or insurance benefits.
After your first visit, Dr. Jones will be able to decide on the course of treatment that best fits your needs and expectations. You will never be pressured to complete more or move faster on a course of treatment than you are comfortable with.
